CMS has defined the QRDA format for the valid and complete PQRS submission: Quality Reporting Document Architecture .

An incomplete QRDA submission process can result in production of inaccurate or non-compliant QRDA records.  The ramifications of this may include negation of any incentives to your EPs as well as the total disqualification of your QCDR, and other collateral regulatory effects.
